About Me
I am a Professor and the Director of the Case Management (CM) Program in the College of Nursing at Samuel Merritt University. I completed my basic nursing education in Japan and, over the past 45 years, have practiced as an RN and taught nursing in both Japan and the US. I have been involved in several projects. The projects related to CM practice are 1) “Multi-Generational Workforce Workshop” with Sutter Health, 2) “Factors Related to Rehospitalization” with Sutter Health, and 3) “Evaluation of the CM Orientation and Mentorship Program for MSN-Prepared New Graduates” with Kindred Healthcare.
In 2005, I and several patient groups established the Japan Chronic Disease Self-Management Association (JCDSMA) to offer Chronic Disease Self-Management workshops. In 2010, I and my colleagues conducted a study to examine the effectiveness of its implementation in Japan. As of March 2021, JCDSMA members have offered 246 workshops (with 2,297 attendees) across 21 prefectures. We have developing partnerships with hospitals, patient groups, local governments, and pharmaceutical companies. Since November of 2020, due to the COVID pandemic, the JCDSMA began to offer virtual workshops.
